As your medication is on repeat, you can use the NHS App to send a request for the surgery to send the prescription to your preferred (nominated) chemist.

You can register for the NHS app online and you do not need to come to the surgery at any point in the process.

Alternatively, request repeat medication via a form that you email to us

Send Online Consultation to Request a Medication Review

Why have I been asked to come in for a review?  What will happen if I don't attend?

All long-term medications need monitoring , at least annually, to check that the medication remains right for you. To make this easier to remember we try to coincide the annual review with your birth month. Please ensure you book an appointment with a member of the practice team if requested for your medication review. Depending on the medications and condition it may be necessary to arrange blood tests or BP checks as well.

If you do not attend for review appointments when called but request medication, then the request will be flagged to a clinician to assess.   A clinical decision will then be made regarding the risks of continuing the medication versus providing a shorter course to allow monitoring to occur versus discontinuing the medication. To avoid this it is really important that you attend for review appointments when called.  "
