Online Access

If you’re a patient at our practice you can now use the new NHS App, a simple and secure way to access your medical record, prescriptions and a range of NHS services on your smartphone or tablet.

You can register for the NHS app without visiting the surgery. All identity checks are carried out online, in the app.

You can use the NHS App to:

• check your symptoms and get instant advice
• book appointments
• order repeat prescriptions

If you already use patient online you can continue to use it. You can use the NHS App as well.

If you are unable to use the NHS App, or wish to request more detailed record access, you can complete a printed application form and attend the surgery in person to verify your identification.

On request, we offer full access to your medical records (from the date of request).

Given the sensitive nature of the information that will be divulged, you will need to attend in person to confirm your ID and complete the below form.
